Centering Experience, Driving Change: A New Vision for Diabetes Care

The Type 1 Diabetes Community Fund provides travel fellowships to grantee partners to engage in and shape global discourse on diabetes care. Grantee partners share reflections on the conferences they attended, from Copenhagen to New Delhi.
The Type 1 Diabetes (T1D) Community Fund invests in the power of local communities for a future where people living with type 1 diabetes can lead healthy and fulfilling lives.
Powered by Panorama Global with seed funding from The Leona M. and Harry B. Helmsley Charitable Trust, The T1D Community Fund supports community-based organizations around the world that serve individuals and families impacted by type 1 diabetes.
In 2025, The T1D Community Fund is awarding $1.3 million to 26 community-based organizations across Africa, Central and South Asia, and Latin America. Since 2022, The Fund has invested nearly $3 million in 56 organizations across 37 countries that provide direct patient care, advocate for improved health policies, and expand capacity-building and education programs in their communities.
Community-based organizations provide vital services and have unique insights into the challenges of existing health systems and the barriers faced by people with type 1 diabetes. Yet, their perspectives are often not included in national or global type 1 diabetes planning.
We are thrilled to support organizations working on the frontlines to close gaps in essential health services and related care for people living with type 1 diabetes. The T1D Community Fund grant recipients provide life-saving health services and programs for people living with type 1 diabetes and their families through five types of support:
Adventist Development and Relief Agency (ADRA) Malawi's purpose is to serve humanity by promoting justice, compassion, and sustainable development, including support for people living with type 1 diabetes. The organization provides relief and assistance in over 120 countries, partnering with communities to create lasting positive change.
The African Community Center for Social Sustainability works with vulnerable people in resource-limited settings through provision of medical care, education and economic empowerment to create long lasting change that is owned by the entire community. ACCESS partners with the Uganda Ministry of Health to care for people with NCDs including type 1 diabetes.
AL-Farouq Charity Society for Orphan's Care is dedicated to improving the well-being of vulnerable communities, particularly orphans, low-income families, refugees, and people living with type 1 diabetes in Irbid, Jordan. Through a holistic approach that combines healthcare, education, and social empowerment, we strive to create lasting impact and improve quality of life.
Ark Development Organization (ADO) is a health education, governance, and environmentally focused organization committed to improving the lives of deprived women, children and the disabled through capacity building, evidence-based advocacy, and participatory innovative programs. The organization’s diabetes support program minimizes health crises through early detection of type 1 and other diabetes and early uptake of treatment through community education, screening, linkage to treatment services, and case monitoring.
Ashuja'an is a leading non-profit organization for type 1 diabetes prevention and treatment in the Arab community, serving over 10,000 children and adults in Israel and Palestine. The organization aims to eradicate the stigma that prevents children and youth living with diabetes to effectively cope and manage through educational community activities and approachable health monitoring measures that normalize diabetes in patients’ families and communities.
Asociacion Creciendo con Diabetes' mission is to improve the quality of life for children, young people, and families living with type 1 diabetes in Guatemala. They provide comprehensive support through education, access to medical supplies, and emotional care, empowering individuals to effectively manage their condition.
Founded by parents of children with type 1 diabetes, CUI.D.AR’s mission is to improve the quality of life of people with diabetes. They work to minimize the impact of diabetes on the lives of children, young people, and adults through prevention, social awareness raising, defense of rights and education. CUI.D.AR's work spans research, education, advocacy, awareness campaigns, incidence and design of public policies, and capacity building. It is the largest community of people living with diabetes in Argentina, and specifically the most representative in type 1 diabetes.
DIA is a civil society organization in the Republic of Moldova that contributes to increasing the quality of life and life expectancy of people living with insulin-dependent diabetes by addressing the right to health, social protection, social inclusion.
The Association for the Fight Against Diabetes and Associated Diseases (ALD) aims to raise public awareness about diabetes and to train and equip physicians in its management. Headquartered in N'Djamena, Chad, ALD provides insulin and monitoring equipment to children living with diabetes through Life for a Child.
ADIC provides socio-medical care to everyone living with diabetes in the Democratic Republic of Congo. They educate people living with diabetes about prevention, nutrition, management, and distribute medical supplies.
Since 2007, ADPC has been dedicated to transforming the lives of people with diabetes in Poços de Caldas through public policies, diabetes education, and compassionate care. In partnership with the community and the public sector, we work to ensure access to appropriate treatments, psychosocial support, and activities that strengthen community bonds. Our goal is to promote autonomy, well-being, and social inclusion for individuals of all ages, with initiatives aimed at enabling a full and integrated life in society, providing essential medical and social support for patients and their families.
B.P. Koirala Institute of Health Sciences, based in Dharan, Nepal, aims to become a center of excellence in patient care, education, and health sector research, including support for people living with type 1 diabetes.
Baitambogwe Community Health Care Initiative (BACHI) contributes to the well being of communities through provision of quality medical and preventive health and social economic services, including serving people living with type 1 diabetes.
The Burundi Non-Communicable Disease Alliance (BNCDA) is committed to improving the lives of individuals with type 1 diabetes (T1D) in Burundi and beyond. Acknowledging the specific challenges faced by those in low- and middle-income countries, BNCDA works diligently to provide equitable access to essential care, education, and resources for children and families affected by T1D.
Center for Rights Advancement and Sustainable Development (CREST) is dedicated to transforming the lives of people living with type 1 diabetes in underserved communities across Nigeria. They promote access to high-quality treatment, comprehensive medical care, and essential support services for people living with type 1 diabetes.
Diabetes Association of Botswana is a not-for-profit NGO that provides support, education, and advocacy for people living with diabetes in Botswana through awareness campaigns, screenings, and youth camps. The organization also works with the Ministry of Health and other partners to support healthcare worker training and education about diabetes to further enhance care for those living with diabetes in Botswana.
DASL is an NGO in Sri Lanka that provides clinical, education, screening, laboratory services, and advocacy to reduce prevalence and prevent diabetes. The headquarters, National Diabetes Centre, provides affordable screenings and free of charge medical supplies and treatment to underprivileged youth living with type 1 diabetes.
Diabetes Awareness Trust (DAT) is a non-profit organization established on March 2017 to enlighten Kenyans on diabetes, including healthy living to enhance the management and prevention of diabetes and other non-communicable diseases (NCDs). DAT aims at reducing new cases of diabetes and NCDs through information sharing of information, advocacy for quality health care, and networking.
Diabetes LATAM is an NGO and foundation based in Panama and serving all of Latin America. Their main objective is to provide education, support, and advocate for people living with type 1 diabetes and their families.
Diabetes Palestine (DP) supports people living with diabetes in cooperation and coordination with care providers. Leveraging community-based approaches, DP provides comprehensive diabetes care with a focus on self-management and education to communities in West Bank and Gaza.
In 2004, Diabetes Society of Maldives (DSM) established a free clinic that provides diabetes services—including type 1 diabetes education, complication screening, and care at no cost to patients. DSM also acts as a center of information on diabetes in the Maldives by educating the community through screening programs, presentations, and health education.
The Diabetic Association of Bangladesh's (BADAS) mission is to improve the lives of children, adolescents, and young adults with type 1 diabetes through comprehensive, multidisciplinary care. BADAS prioritizes optimizing glycemic control, promoting healthy lifestyles and balanced diets, supporting mental well-being, and ensuring regular check-ups with annual complication screenings.
The Diabetic Association of Pakistan provides medical care, education, and rehabilitation for people with diabetes, regardless of their background. They aim to improve lives through patient care, education, and strong leadership in healthcare.
Diabetic Children Foundation improves the lives of children with type 1 diabetes and their families. The foundation focuses on community awareness and stigma reduction, education, and access to technology for type 1 diabetes care and treatment.
The Ethiopian Diabetes Association was established in 1976 to provide a platform for people living with diabetes to provide information and prevent treatment and related problems. The association was founded by the Ethiopian Medical Association, health care professionals, and people living with diabetes.
Fondation Diabete MNT contributes to the fight against diabetes and other NCDs in Guinea. The organization provides education, training for healthcare workers, and screenings to increase awareness and improve prevention and early detection of diabetes. They also advocate for increased access and quality of care.
The Aprendiendo a Vivir con Diabetes Foundation (FUVIDA) was established in 2006. It brings together people with type 1 diabetes, providing them with education and continuous treatment. FUVIDA is a recognized center by hospitals and health centers, which refer newly diagnosed children and young people with diabetes. The center trains both healthcare professionals and individuals with diabetes.
The Fundación Diabetes Juvenil del Ecuador’s mission is to improve the quality of life for people with type 1 diabetes in Ecuador. They provide nutrition education, counseling, and peer support to people with type 1 diabetes, parents, or caregivers, and in schools.
Fundación Los Fresnos Casa de la Diabetes is an organization that works in Ecuador, its objective is to provide support to people living with diabetes or at risk of suffering from it. We work to educate people with diabetes and the community through education programs and workshops and medical-social interventions, training for health professionals, development of clinical practice guidelines, research, development of educational material, among others. We manage support programs, provision of supplies, comprehensive medical services, and camps for people with type 1 diabetes.
Generation for Environmental Education and Enhanced Renewal (GREENER) is a women-led non-profit dedicated to improving the lives of children, mothers, and women living with type 1 diabetes in vulnerable and marginalized communities in Southeast Nigeria.
The Geraldine Trada Foundation is a charitable organization based in Rwanda, dedicated to raising awareness of type 1 diabetes. Through comprehensive education programs, patient-centered support, and advocacy, the Foundation strives to improve the lives of individuals living with diabetes.
Guerreros Azules advocates for diabetes care for children with type 1 diabetes in Venezuela. Their programs promote health services, education, recreational activities, and supplies and resources that support, treat, and save the lives of children in vulnerable conditions.
The Haifa Association for Children with Diabetes is dedicated to supporting children with diabetes and their families. Their mission includes providing healthcare resources, essential medical supplies, education, and community assistance to improve the lives of these children, especially in the challenging conditions of Gaza. The Association works to ensure that diabetic children receive the care and support they need to manage their condition and lead healthier, more resilient lives.
Health Poverty Action acts in solidarity with health workers, activists, and communities worldwide to improve health and challenge the causes of poverty, including serving people living with type 1 diabetes. We do this in three distinct ways: we approach health as an issue of social justice; we prioritize those missed out by others; we address the full range of factors which impact on health.
Innovations for Development (I4DEV) promotes and defends the health rights of current and future generations of Ugandans using innovative solutions, engaging communities, and delivering quality services while focusing on the most marginalized. Through this lens, the organization responds to the needs of people with type 1 diabetes through community building, health system strengthening, and advocacy.
Kabukye Trust works to motivate youth facing rural and financial challenges. The organization's type 1 diabetes initiative, Sugarwise Starlets, focuses on screening, design and standardization of care protocols, education, and advocacy for children and adolescents living with type 1 diabetes.
La Maison Bleue du Diabète is at the forefront of diabetes care for children, adolescents, young adults, and adults with type 1 diabetes living in the Republic of Congo. The association provides free medical consultations and insulin delivery to patients through two medical centers in Brazzaville and Pointe Noire. In addition, the association works to support the diabetes community in Congo through education, healthcare capacity building, and advocacy.
Let's Be Transformed (MAGUFINA) serves vulnerable communities in resource-limited settings by providing comprehensive medical care, education, and economic empowerment to create sustainable, community-owned change. They specialize in delivering integrated healthcare solutions for refugees and asylum seekers living with non-communicable diseases, particularly type 1 diabetes, in Dzaleka Camp, Malawi.
Meethi Zindagi is a nonprofit organization that aims to improve the quality of life for people living with type 1 diabetes in Pakistan through support, empowering education, awareness and advocacy. The organization provides insulin support, access to medical care, structured education and emotional wellbeing through peer support.
Midlands Diabetes Interest Group raises awareness among people with diabetes through education and advising local communities. They offer comprehensive care of complex cases of diabetes and preventive management for patients with diabetes, and attention to pre-diabetic conditions in the Midlands province.
Nityaasha Foundation supports children with type 1 diabetes from less privileged backgrounds by providing free medical aid (insulin and glucose strips) and pathology tests. At health centers and periodic home visits, Nityaasha staff closely monitor and educate families on proper insulin storage, insulin injection technique, injection sites, and daily blood glucose tests, and records the result. The team also provides information on type 1 diabetes day-to-day management to lead a normal quality of life.
The Pediatric Association of Liberia 's vision is to have a nation where every child in Liberia can grow to achieve his or her full potential. The Association will lead the charge, working with other professional associations, partners, and individuals in promoting the health of newborns, children, and adolescents in Liberia, including those living with type 1 diabetes, through advocacy, dissemination of information and execution of targeted programs that will improve the life of the Liberian child.
Pillar of Health’s mission is to have every child/youth living with type 1 diabetes in Rwanda have health insurance. They help raise funds to provide free health insurance to those who cannot afford it and connect them with local solutions to pay for themselves. They also work with the Government of Rwanda to provide insulin and other supplies to district hospitals.
Our mission is to support children and adolescents with type 1 diabetes and their families by providing access to vital information, assistance, and resources that improve their quality of life, integration into society, and access to comprehensive education. We strive to create conditions where every child with diabetes can reach their full potential, regardless of the limitations associated with their condition.
Reconciliation and Development Association (RADA)'s mission is to support community development by building bridges to sustainable solutions inclusively in health -including type 1 diabetes - education, agriculture, technology, peace, and environmental protection.
Renel Ghana Foundation is a non-governmental organization working to promote and protect the rights of women, youths, children and persons with disabilities, as well as people living with type 1 diabetes.
Rwanda Diabetes Association is a nongovernmental organization that aims to improve the well-being of people living with diabetes in Rwanda through education, training healthcare workers, and access to care. The organization promotes diabetes care and prevention, supports people living with diabetes and their families, and bolsters global advocacy efforts.
Samatvam: Science and Research for Human Welfare Trust is a group of institutions that treats and manages diabetes and other endocrine and metabolic disorders. The goal is to provide health care of the highest possible quality to the highest possible quantity— to foster humane, honest, and holistic health care.
Sweet Life Diabetes Community empowers people with diabetes on their journey to a healthy, happy life by providing answers to their diabetes questions and a community to support and share their experiences. Sweet Life serves people living with type 1 diabetes in South Africa as the country’s largest online diabetes community.
Tanzania Diabetes Association aims to educate the community in Tanzania about diabetes and to support research, education, and care of people living with diabetes. The organization promotes affordable quality care and develops models of care and interventions that include early diagnosis, availability of insulin, and continuous care and monitoring.
St. Francis hospital Nsambya, based in Kampala, is a private not-for-profit hospital under the the Registered Trustees of Kampala Archdiocese. The hospital provides sustainable, high-quality healthcare, training, and research while ensuring access for the economically disadvantaged. St. Francis hospital Nsambya, through various partners, supports type 1 diabetes care by providing essential T1D supplies, training health workers in T1D management and equipping T1D centers at the hospital and beyond.
UMID Charity Community Association for the Rights of Patients with Diabetes and Disabilities Karakalpakstan Republic Branch's aims to support and protect the rights of individuals with diabetes and non-communicable diseases (NCDs). The organization works to enhance their social well-being through assistance and charitable activities that improve the quality of life for patients with disabilities and those living with NCDs
The Union of Associations of Persons with Diabetes of Macedonia is a civil society organization that seeks to improve the quality of life for people living with diabetes in North Macedonia. The organization advocates for better health services, provides education for individuals and families, and raises awareness among the public.
Through its Health and Nutrition program, VEReF Development Network focuses its type 1 diabetes efforts on patients, caregivers, and community education in addition to screening and awareness raising. By improving patient outcomes, VEReF Development Network's services support overall individual and family well-being.
UMID is a nonprofit diabetes information organization advocating for people with diabetes in Uzbekistan. UMID assists in improving the quality and life expectancy of people living with diabetes through education, providing access to quality medical care, and protecting the rights of people with diabetes.
